New US Coast Guard Casualty Reporting Form

On 1 July 2016 the US Coast Guard released a new version of its "Form CG-2692" covering reports of marine casualties, commercial diving casualties, and outer continental shelf-related casualties.

This form is the result of an extensive initiative to overhaul the previous form, which had remained unchanged for nearly 40 years.

The new version of the form encompasses several changes, including:

  • Revised addendum forms for barge involvement, personnel casualties, witnesses and chemical testing;
  • Streamlined data fields to align with statutory and regulatory language; and
  • Ability to be filled out entirely electronically, including the use of a digital signature.

The new form is available on the US Coast Guard Homeport website or via the links attached.

The new form will be used for all reports of marine casualties as of 1 July 2016. The report must be submitted within five days of a marine casualty and should always be reviewed by Skuld's local legal correspondents prior to submission to the Coast Guard.
